Unable to view the survey in editing or view mode?

Hi i did up the survey part in the programme but i cant be able to view the survey at all in edit or view mode. HELP!!!

    Hi there! Please make sure your web browser is at "Actual Size" (100%). If it is zoomed in or out, the Survey widget may not appear on the screen. Once you set your browser to 100, please refresh the page.
