certificate not displaying my name or name of course completes

Hi, I recently completed the ADOM Safe Ministry Essentials Level 1 course, and the certificate issued doesn't contain my name or the name of the course. I also did Level 2 and did not have any problem with that certificate, it displayed correctly. Thanks
